| Long
Term Disability Insurance (LTD) |
In lieu of collecting State Disability Insurance payments from
employees each month, the City offers a richer voluntary Long Term Disability
Insurance group policy. All eligible full and part-time benefited employees
are guaranteed enrollment into the LTD group policy options within the first 30 days
of employment. Paid claims under this policy can equal as much as two-thirds
the employee’s gross monthly salary and may be paid up to age 65 (under
SDI,
disability claims payments are not payable beyond 52 weeks from the date of
disability). |

| Long
Term Care Insurance (LTC) |
Long term care group insurance is available
to all full and part-time benefited employees. Long
term care provides coverage for home, nursing home,
or assisted living facility care. Guaranteed enrollment
is available for eligible City of San Jose employees
within the first 90 days of employment.
Late enrollees and eligible family members such
as spouses, domestic partners, parents, parents
in-law, grandparents, and grandparents in-law are
free to apply at any time, but are subject to underwriting
review and approval. |

| Accidental
Death & Dismemberment Insurance (AD&D) |
The City of San Jose offers group accidental death and dismemberment
insurance (AD&D) with identity theft protection to all full and part-time benefited employees.
Individual and family coverage is available to provide financial
security for employees and their families should a participant
suffer an untimely accidental death or survive a severe accident. |

| Medical
Reimbursement Account (MRA - Pre-tax Benefit) |
| The City of San Jose offers employees the opportunity to contribute to medical reimbursement account (MRA) administered by Allstate-New Liberty. Full and part-time benefited employees are eligible to contribute as much as $2,500 per year on a pre-tax basis for eligible medical expense reimbursements such as office visit and prescription co-pays, contact lens supplies, etc. |

| Dependent Care Assistance Plan (DCAP - Pre-tax Benefit) |
| The City of San Jose offers employees the opportunity to contribute to a dependent care assistance program (DCAP) administered by Allstate-New Liberty. Full and part-time benefited employees are eligible to contribute as much as $5,000 per year on a pre-tax basis for qualified child care or qualified elder care expense reimbursements. |
