SUBJECT: AGREEMENT
WITH CLUB ONE, INC.
RECOMMENDATION
Approval of an agreement with CLUB ONE, Inc. to provide a comprehensive Wellness Program for the San Jose Fire Department in an amount not to exceed $211,100 for the period
July 1, 2001 to June 30, 2002.
BACKGROUND
Since FY 1999-2000, on-going funding has been approved to implement an annual comprehensive Wellness Program in the Fire Department for all uniformed and non-uniformed employees. A contract was executed with CLUB ONE, Inc. to provide this program in 2000-2001. CLUB ONE, Inc. has not only been able to meet the deliverables of the program but more importantly, gain the trust and confidence of the employees of the San Jose Fire Department.
ANALYSIS
A Request for Proposal was initiated to solicit other potential contractors in the Wellness field for 2001-2002. CLUB ONE, Inc., was selected to continue as the FY 2001-2002 Wellness provider. CLUB ONE, Inc. was selected because it was the only responder meeting all RFP requirements who submitted a formal response. Under this agreement, CLUB ONE, Inc. will continue to provide a full time staff coordinator to manage the program, and will also be responsible for continuing to develop and maintain a fitness information database and comprehensive reporting system. In addition, CLUB ONE, Inc. will work with the Fire Department’s Employee Assistance Committee and Critical Incident Stress Management Team.
The major requirements are for the contractor to oversee the Wellness Program as follows:
• Act as a full-service contractor for the program and provide a full-time coordinator
• Produce and deliver a health education and wellness program promotion component
• Deliver personnel physical fitness testing and testing equipment acquisition
• Deliver fitness training at job sites and a train-the-trainer fitness instructor program

• Provide a fitness information database management system and comprehensive reporting system
• Provide an incentive awards system
• Ensure coordination with the Employee Assistance Committee and the Critical Incident Stress Management programs
• Provide a maintenance program for all departmental fitness equipment
• Provide an injury management component integrated with the Wellness Program
• Develop a job related physical fitness protocol component
The agreement with CLUB ONE includes all of the major requirements listed above and will be for a period of one year.

This agreement has been coordinated with the Budget Office and the Office of the City Attorney.
COST IMPLICATIONS
$211,100 has been included in the 2001-02 Proposed Operating Budget to support this agreement.
