SUBJECT: Municipal Health Services Program Provider Agreements Supplemental Memorandum

Since the date of our initial memorandum with respect to this item, the County of Santa Clara has requested a modification in the format of the agreement to extend the term for participation in the Municipal Health Services Program. We are requesting that the City Council adopt a resolution authorizing the City Attorney to negotiate and execute a new provider agreement with the County of Santa Clara. The City Council had previously adopted a resolution in 1999 approving a second amendment with the County of Santa Clara extending the term through June 30, 2001. However, the second amendment was never fully executed. The County of Santa Clara now wishes to enter into a new agreement retroactive from the period January 1, 2000 to June 30, 2005.
