To complete
a PDF-formatted form above in Adobe Reader version 7 or higher:
1. Save a Copy.
2. Open it in Adobe Reader.
3. Click the Typewriter Tool button.
4. Click with the Typewriter Tool to place text anywhere on
the page.
5. Move typed entries by clicking to select the text and dragging
it by its border.
6. Click again with the Typewriter tool for each answer you
type.
7. Save/Print.
