Visit the Benefits Fair on October 24, 2018 from 10:30 AM - 2:00 PM in the City Hall Rotunda! The focus of this year's fair will be on the changes to our health plans, while continuing to highlight the benefits of wellness.

The City of San José currently offers a choice of seven medical insurance plans:
 
  • Kaiser Permanente $25 Co-pay HMO
  • Kaiser Permanente $1,500 Deductible HMO
  • Kaiser Permanente HSA Qualified $3000 Deductible HMO
  • Sutter Health Plus $20 Co-pay
  • Sutter Health Plus $1,500 Deductible Plan
  • Sutter Health Plus VISTA $2,500 Deductible Plan
  • Blue Shield $100 PPO

Medical Rates

A Health In-Lieu plan is available for employees with alternate group medical coverage.  For more information, see the details in the Health In Lieu Plan Document.

 City of San José Open Enrollment for Life and LTD Insurance

During open enrollment period, October 22 – November 9, 2018, City of San José employees will have a special one-time opportunity to elect Life and Long Term Disability (LTD) insurance without submitting medical history.

Open Enrollment Highlights 

• You may apply for Plan 3 Additional Life coverage up to $200,000

• You may apply for Plan 3 Dependents Life coverage up to $20,000 for your spouse or domestic partner

• You may enroll in the LTD-60 Plan

• Medical underwriting approval is required if you previously submitted medical history that was not approved by Standard Insurance Company (The Standard).

Enroll in Plan 3 Vantage Life and LTD-60 insurance today. 

These policies have exclusions, limitations, reduction of benefits and terms under which the policies may be continued in force or terminated. Please contact The Standard or your employer for additional information, including costs and complete details of coverage.

Affidavit of Domestic Partnership for 2019!

Per Internal Revenue Code Section 152(a) and IRS Publication 501, employees are required to reconfirm non-taxable domestic partnership status annually. The 2019 CSJ Affidavit of Domestic Partnership form must be completed to continue covering your domestic partner and domestic partner's children as non-taxable.

Failure to submit the 2019 Affidavit of Domestic Partnership will result in a status change from non-taxable to taxable effective January 1, 2019.
