The Day Rehabilitation Program at Grace Community Center

The Day Rehabilitation program offers mental health consumers a more structured and goal-oriented level of participation at Grace. The Day Rehab Staff provide support and guidance, helping clients develop and achieve individualized goals that support healthy, independent living.  Clients work closely with Day Rehab Staff to design a unique treatment strategy utilizing the various creative, experiential, educational and social programs offered at Grace to achieve their individual goals.

How to Enroll in the GCC Day Rehabilitation Program

• Contact Grace Community Center for an Introduction to Day Rehabilitation during which time you will receive a Day Rehabilitation Information Packet, containing the GCC Membership form, a Release of Information, and Consent for Treatment.  Each must be completed during the introduction to the program.  Please contact the Day Rehabilitation Coordinator for more information at (408) 293-0422.

• New clients to Grace Community Center will begin with a four-week orientation phase, where the client attends a minimum of three days per week, to assess his/her eligibility and commitment to the program.  During this phase the client will be assigned a Day Rehab Specialist, with whom he/she will work closely to develop individualized goals.

• To complete the enrollment process, the Day Rehab Staff will need to work closely with the client’s service team, including the coordinator, rep payee, and/or conservator.
