The Healthy Neighborhood Venture Fund (H.N.V.F.) program was established by the City Council in 2000 and funded by the Tobacco Master Settlement Agreement, which is an agreement between certain cities (including San José), 46 States and the major tobacco companies regarding the marketing and consumption of tobacco products. Through this agreement, the City of San José will receive millions of dollars in general revenue from the tobacco companies for unrestricted purposes. Historically, $9-$11 million has been received annually. The funds have been allocated to youth and senior programs.
The H.N.V.F. Leadership Committee consisting of 2 Council members and 5 community members is responsible for: 1) setting policy direction; 2) facilitating interagency collaboration; 3) evaluating emerging trends; 4) assisting in advancing the goals of H.N.V.F. Strategic Work Plan; and 5) making recommendations for disbursement of H.N.V.F. grants.
2009-2010 SELF CERTIFICATION AND SCHOOL AGREEMENTS

As part of the Exhibit F requirement for your programs, please submit a self-certification letter on your agency’s letterhead following the format on the link below. If you have submitted one already, please disregard this request.
For those agencies who conduct services on school campuses, please provide an operational agreement or any form of agreement signed by an authorized signator and the school(s) districts which will indicate permission to use their facilities. This requirement is specified in your grant agreement, Exhibit E #6. Please contact your HNVF Analyst should any questions arise.
