MTL Work Request Form
 

Taking just a few minutes of your time to complete the MTL work request form will give us the information we need to expedite your request. Please complete all information on the form.

An online version of the work request form is available here. A pop-up window will appear after you check the box of the service(s) you require. This window will list any documents or other information the MTL will need to complete your request, as well as a brief description of the service.

Print the completed form and send your request with any necessary documents to either Alex Shkouratoff (construction services) or Richard Lucas (geotechnical services) at:

A Special Note Regarding Materials Submittals:
• ALL submittals MUST be accompanied by a work request form. If a completed form (including charge number) is not attached, your submittal review will be delayed!

• Direct all submittals to Alex Shkouratoff at the MTL, even if it is a re-submittal. For your convenience, submittals may be faxed, routed or hand-delivered to the MTL. However, for faster service we recommend faxing or hand-delivering submittals to the MTL.

• The MTL will complete your submittal review within 10 working days, unless rush service is requested. For rush service, specify the date needed on the work request form. Please be aware that during busy periods, overtime may be required to complete a "rush" submittal.
