Regulatory Permit Forms

Some business services need a permit. This includes running an amusement ride, holding a carnival, or handing out flyers.

You must fill out a permit form every year and each time you renew. After the form and payment are received, the permit will be mailed to you. In some cases, you may also need to provide extra documents or get approval from other departments.

Please select from categories below for additional forms needed (if applicable)

According to San José Municipal Code Chapter 6.36, the owner or renter of a property must submit a written notice if they don’t want to receive any handbills at their property. The properties that have notified the City will appear on the handbill objectors list. So, no one is allowed to leave handbills at these properties.
