SJFD Medication Tampering Notification
On April 14, 2025, the San José Fire Department identified signs of tampering involving controlled substances during a routine inspection at a fire station. Steps were taken to secure the affected inventory, notify law enforcement, initiate a department-wide inspection of all controlled substances, and remove any items showing signs of possible damage or tampering.
Since then, an arrest has been made. The San José Fire Department is cooperating with the ongoing investigation being led by the San José Police Department. We take this matter seriously and remain committed to transparency and accountability.
Out of an abundance of caution, the Fire Department has isolated the date range of October 1, 2022, through April 18, 2025, as the period during which tampering may have occurred. Patients who received care from the San José Fire Department during this timeframe may request a free copy of their Patient Care Report (PCR) if they would like additional information about their treatment.
Any individual who receives care from the San José Fire Department may request a copy of their PCR through the Public Records Request portal. For patients treated between October 1, 2022, and April 18, 2025, the standard $15 processing fee will be waived.
Individuals may also request information by calling (408) 535-8511. The phone tree provides recorded instructions on how to request a PCR in English, Spanish, Vietnamese, and Mandarin Chinese. Callers may also leave a voicemail to request information by mail or email.
