COVERED DEPENDENTS: WHAT YOU NEED TO KNOW
The City of San José requires eligibility documentation for all individuals who are enrolled as dependents of City employees in the medical, dental, vision, employee assistance, and/or any in-lieu plans. Please refer to the documents below to understand how to meet this requirement.
Life Events: Mid-Year Benefit changes
Log into eWay to initiate mid-year benefit changes outside your new hire/newly eligible event or annual open enrollment. i.e. To request changes to your own or dependent's benefit coverage due to a qualifying life event, such as change in marital status, birth or placement of a child, and/or gain or loss of benefit coverage or eligibility. Life Event benefit changes must be submitted in eWay within 30 days of the qualifying life event. Required proof of event and dependents is not required to request the change, but must be uploaded in eWay or provided within 60 days. Find how to guides on the Life Event website.

Affidavit of Domestic Partnership
Use this form to provide proof of relationship for a current or new domestic partner that is or will be covered as a dependent on your City benefits.
Affidavit of Termination of Domestic Partnership
Use this form to provide proof that your current should no longer be covered as a dependent on your City benefits.
Use this form when submitting mid-year Life Event Benefit changes in eWay. This document serves as a promissory note that you will to provide the required documents within 60 days of your Life Event if you are not able to upload the required the proof of event or dependents(s) upon submission.
Wellness Rewards Enrollment/Change Form
Use this form to enroll in or update enrollment in the City's Wellness Reward Program.
