About the Program

The City of San José Department of Parks, Recreation and Neighborhood Services (PRNS) administers the Older Adults (50+) Health and Wellness (OAHW) Grant Program (formerly known as Senior Health and Wellness) and collaborates with several non-profit organizations and public agencies that are also engaged in meeting the needs of the senior population. The OAHW grant program seeks to reduce social isolation and improve the health and wellness of individuals ages 50 and up by providing quality programs at Santa Clara County (SCC) sponsored Senior Nutrition Program (SNP) sites and additional service locations within the City of San José. Through this local grant program, PRNS is able to offer a variety of social services to seniors, their families, and the community.

This Request for Qualifications (RFQ) is designed to create partnerships with organizations to provide quality services to reduce social isolation and improve the health and wellness of individuals ages 50 and up. Qualified agencies will be placed on an “Eligible Service Provider List.” Inclusion on the Eligible Service Provider List does not guarantee that the agency will be selected to perform services for the City. The City will select agencies from the Eligible Service Provider List to grant funds to on an annual basis. Please refer to Part 3, Section 3.3 for details in the General Information and Requirement (GIR) document about the process for awarding funds.

Entry Criteria 

To be considered for the Older Adults (50+) Health and Wellness Grant Program from the City of San José, applicants must meet all ENTRY CRITERIA at the time of submittal. Submitted applications must be responsive to all application instructions and meet minimum eligibility requirements. Agencies who do not meet all entry criteria and minimum eligibility requirements will not be considered and evaluation of the application will NOT be made.

Information Process Description
Eligible Applicant

Applicants must be one of the following entities:

  • A government agency; or
  • A private nonprofit agency with current 501(c)(3) status; or
  • A faith-based entity

Note 1: Applicants are not allowed to use a fiscal agent to apply on their behalf. All applicants must be one of the two entities listed above in order to be eligible for SHW funding.

 

Note 2: Previously funded agencies must be in good standing with the City of San José and understand that past performance may be included in the criteria used to determine eligibility for this funding cycle. Potential applicants should contact Adriana Copado, Older Adults (50+) Health and Wellness Analyst, at Adriana.Copado@Sanjoseca.gov if they are uncertain of their organization’s status. Status is not subject to appeal.

 

Note 3: All organizations will be required to obtain and show proof of a City of San José business license and evidence of active standing with the Secretary of State of California.

Target Population

Projects must serve the following SHW target population as identified

  • Older Adults age 50 and up; and
  • Older Adults that attend Santa Clara County (SCC) sponsored Senior Nutrition Program (SNP) sites located in San José; and
  • Older Adults who are San José residents.
Eligible Service Areas

Services must be provided to at least one Santa Clara County sponsored Senior Nutrition Program Site located in San José. For a current list of Santa Clara County sponsored Senior Nutrition Program sites within San José, please see Part 2, Section 2.2 in the General Information and Requirements document (GIR).

 

Note 1: Applicants may propose to serve one additional site that is a non-Santa Clara County-sponsored Senior Nutrition Program site. The City will determine award based on available funding, the needs of the target population when evaluating the proposed program.

Service Categories

Applicants interested in funding must demonstrate experience in providing services in at least one of the three (3) defined service categories, which are listed and demonstrated in Part 2, Section 2.3 (GIR).

  1. Support Services; and/or
  2. Health and Wellness Promotion and Educational Classes; and/or
  3. Age-Friendly Programming
