Lifesaving Information for Emergencies
The L.I.F.E. File is a place for you to store important medical information that emergency medical personnel (firefighters, paramedics, and hospital staff) need in a time of crisis. By completing the L.I.F.E. File form, you can provide emergency personnel with vital lifesaving information, even if you are unable to speak. It is very important that you keep this information up to date, accurate, and placed in a prominent spot in your refrigerator.
How to Use the L.I.F.E. File:
- Please complete a L.I.F.E. File form, which can be obtained from the Fire Department. Be sure to include the date that you completed this form. This will help emergency personnel know how recent is the medical information. Seek help filling out the form from your doctor as needed.
- Fold the L.I.F.E. File form in half and place in the L.I.F.E. File pouch.
- Place the L.I.F.E. File pouch on your refrigerator door. (It's a magnet)
You may obtain the L.I.F.E. File forms and pouches by stopping by any fire station. Replacement forms can be downloaded from the Santa Clara County EMS Agency.
